Diamond Care Health Service

Through the coordinated efforts of the patient’s physician and DCHS intake nurse, clients will receive an individualized "plan of care". A Plan of Care details the client’s health situation, what service and treatment they will require and which equipment and/or supplies they may need also including the doctor’s expectations as a result of treatment. Clients will receive effective health care in the reassuring comfort of their home. We adapt our services based on each individuals needs and personal situation. As Client’s needs change, we adjust our services accordingly.

Skilled Nursing

A registered nurse will assess the patient and be responsible for teaching you about medication, treatment, your disease and any modification you may need to make to manage your disease. An LVN may deliver skilled nursing under the supervision of a registered nurse.

Physical Therapy

Our hardworking physical therapists will assist the patients with mobility issues by making use of the following services:

Speech Therapy

We have Speech Therapists on standby to assist patients with language and communication problems. Hardworking and devoted, they are more than ready to assist in recovering skills such as:

Occupational Therapy

An Occupational Therapist will guide and instruct you in caring for yourself and activities of daily living to improve your independent functioning. You or your loved one can expect assistance in:

Home Health Aide

A Home Health Aide (HHA) will provide personal care and assist with activities of daily living. They will offer their assistance not just to the patients themselves, but also to the clients' homes as well. Among services rendered are:

  • Doing Groceries
  • Bathing
  • Dressing
  • Grooming
  • Medication Reminder

Medical Social Worker

A Medical Social Worker (MSW) will assist in resolving emotional, social and financial problems which may impede the patient's recovery. They can also link you to programs which may assist you
  • Short term counseling

  • Financial Information and Education
  • Referrals to appropriate community services

The best way to provide the highest quality level of care requires an initial meeting where family and/or client meet with a Visiting Diamond Care Health Service coordinator. This meeting includes a detailed review of care needed, schedule request (if known), specific requests of family or client, and any other concerns. This is an excellent opportunity to ask questions of Visiting Diamond Care Health Service and it is an excellent opportunity for us to get to know you as well.

The following topics are commonly discussed during initial meetings.

  • Specific physical needs of the care recipient with regards to bathing and grooming, dressing, meals and diet, mobility, and incontinence.
  • Reviewing schedules involving sleep, eating and medicine.
    Specific emotional needs such as social interaction, activities, conversation, appointments, and companionship.
  • Specific assistance with homemaking such as preparation of meals, groceries, linens, laundry, and light housekeeping.
  • Logistics of schedules, access to the home, documentation, and the details of how to work with scheduling changes, the office.
